This module allows for 3 columns but only 2 fields appear for content. I have ‘custom’ selected. I did a save and exited the customization area. Coming back to it however does not show the fields for the 3rd column.

Can you check if the No of items is 2? The number field shown depends on No of items? If you have No of items as 3, 3 custom field should show up.
