Not sure if this is a theme problem, or a problem with WordPress core.
Today I noticed that four pages I’ve recently created have been added to my menu at the top of the site.
I do NOT have “Auto Add Pages” check-marked on the Appearance page. Additionally, the pages are NOT listed amongst the menu items which I can modify.
I tried adding one of the errant items, saving the menu, and then removing it. However, that didn’t help.

Somehow the Managed Locations had changed to the default of “blank” after the last update. Once I placed Primary in Primary, it works fine.
Figured I would reply with the “resolution” in case anyone else runs into the problem themselves.
