Javascript for Lightbox Sign Up Form

I’m working with aweber to insert contact forms into my website and I’m having trouble. I have placed this code in the header.php file:

In the body of the page where I’d like a click on an image to trigger I used a ‘Code Embed’ plugin where I created these custom fields:

I inserted the Direct Link Image URL correctly.

When I update the page I get a link on the image but, nothing happens.

Can you help out with this?
