Is there an updated version of Clean Business? There is a jQuery conflict coming from your Clean Business theme, I checked the changelog of your theme, and it was last updated by its developer last October 25, 2018. There have been lots of WordPress updates this year, especially WordPress 5.

WordPress comes with jQuery included in /wp-includes/js/jquery/ and every theme and plugin should load it from there. My other plugins use WordPress default jQuery while your Clean Business theme’s jQuery is outdated, which is causing a conflict. Is it possible to fix this?

We use the wp-include/js/jquery/ in the theme as well. Can you please let me know what conflict you are getting so that I can help you figure out what is causing the conflict. Please post in your site URL as well.

I checked your site both on Google Chrome and Mozilla Firefox and I am not getting any error on my browser console. Can you check if you are having the same issue in different browsers?

That may be due to a plugin you are using. Can you try deactivating all your plugins and check if the issue persists?

I can try that, but why would a plug-in be only a problem with this theme and not others?

Yes, it persists with all plugins deactivated. FYI the survey is done through Thrive Quiz Builder from Thrive Themes which is a plug in itself. I deactivated all other plugins to rule them out. Quiz builder works with all other themes that I have except Clean Business and I like Clean Business so I want to keep it.

Did you deactivate Thrive Quiz Builder? I think this is a conflict between the plugin and the theme.

Yes I deactivated The Quiz Builder. Obviously there is a conflict, but the conflict is the JQuery one which was shown to you. This theme is the ONLY theme I have from any developer giving the Error. ALL other themes work with the Quiz Builder Plugin.
