MailChimp for WordPress Lite on Catch Box

Hi guys, i tried to use MailChimp for WordPress Lite on Catch Box theme Widget and I haven't had any luck. It keeps saying "Please fill in the required fields." even though i put the email address there. I've watch instructions on YouTube and followed everything and still no luck. Any help would be greatly appreciated. Thank you.
