How to Disable Social Icons Appearing in Pages

Hello. My pages are showing social icons and a "Leave Comment" box. How do I get rid of these? I've looked everywhere and can't seem to find the place to do this. Also, in Contact Us I do not see the form I prepared. Any ideas why this is? The website is: mychurchconnected.com Thanks for your help with this.
