Header logo missing on 1 specific page

Hi, I have 2 issues on this specific page containing forms : - the header logo doesn't appear - the "mini menu" (id="header-right-menu") doesn't appear when you scroll down The page is : http://cathleennaundorf.com/contact/ Can you help me fix this please ?? :-) Thank you very much!
