Contact Form plugin code not working

I am having problems with Contact Form code not working in my site - http://luxuryrentalcars.net.nz/contact-us-booking-form/ I have tried 2 different plugins, Contact Form7 and Form Maker (currently activated). I have also tried installing the code on a new page. Still no joy. Can you help please?
