Contact Form 7 mobile form

Hi Sakin, I created a Contact Form 7 which looks great in desktop but not in mobile http://summer-in-utah.com/request-quote-2/ Is there a way to use a conditional statement to select another form designed for mobile if the screen size is max-width=400? I realize I can use the @media query for formatting, but I would like to call a different form. thanks Lisa
